If you've ever asked your doctor to check your cortisol because you feel exhausted by 2pm but can't fall asleep at 10pm, you've probably heard some version of: "Your cortisol is normal." And then... nothing. No explanation for why you feel like you're running on fumes and adrenaline at the same time.

I hear this constantly in my practice, often from the same patients who've already been through the thyroid workup and the cholesterol panel and been told everything is "fine." Cortisol is usually the next stop, and it's usually just as frustrating - because a single cortisol number, drawn once, at one point in the day, almost never tells the whole story.

## What a Single Cortisol Draw Actually Tells You

Cortisol isn't a static number like a cholesterol level. It's a rhythm. In a healthy pattern, cortisol should be highest in the morning,  giving you that natural, alert "get up and go" feeling,  and then taper gradually throughout the day, hitting its lowest point at night so your body can wind down and sleep.[ ](https://pubmed.ncbi.nlm.nih.gov/39177247/)[https://pubmed.ncbi.nlm.nih.gov/39177247/](https://pubmed.ncbi.nlm.nih.gov/39177247/)

A single blood draw, usually done in the morning, captures one snapshot of that curve. It can rule out the two extremes : Addison's disease (dangerously low cortisol) and Cushing's syndrome (dangerously high cortisol)  but it says almost nothing about whether your daily rhythm is actually working the way it should.

That's the gap. Most people who feel "wired and tired" don't have a disease-level cortisol problem. They have a pattern problem — and a single number can't show you a pattern.

## The Patterns I Actually Look For

When someone comes in with that classic wired-but-exhausted feeling, I'm usually looking at a four-point cortisol curve throughout the day (morning, noon, evening, and night), often alongside DHEA, because the two are made from the same precursor and tend to move together under chronic stress.

A few patterns show up again and again:

The flipped curve. Cortisol is low in the morning (hence the fog and the need for coffee before you can function) and elevated at night (hence lying awake replaying your day at 11pm). This is one of the most common patterns I see, and it's almost never caught on a single morning blood draw because a flipped curve can still land in a "normal" range at any single timepoint.

The flattened curve. Instead of a strong morning peak and a steady decline, cortisol stays low and relatively flat all day. There's no real spike to get you going, and no real dip to let you rest. This pattern is common after a long stretch of chronic stress, the body down-regulates the whole system rather than swinging wildly.

The exaggerated curve. A big morning spike, a steep crash by midafternoon, and a second, smaller spike in the evening just as you're trying to relax. This is the pattern behind the 3pm wall so many of my patients describe, along with that unwelcome "second wind" right before bed.

Low DHEA relative to cortisol. When the adrenal system has been under sustained demand, DHEA often drops faster than cortisol does. Since DHEA supports resilience, mood stability, and healthy aging, a low DHEA-to-cortisol ratio is often part of why someone feels depleted even when their cortisol technically sits in range.

## Why This Isn't Just "Adrenal Fatigue"

I want to be clear about something: I'm not diagnosing "adrenal fatigue." That's not a recognized medical diagnosis, and your adrenal glands themselves are almost never the actual problem. What's really going on is upstream — in the communication between your brain (the hypothalamus and pituitary) and your adrenal glands, a feedback loop called the HPA axis.

Chronic stress, whatever the source (poor sleep, blood sugar swings, unresolved gut issues, overtraining, or simply too much cumulative demand for too long)  doesn't damage your adrenal glands directly. It disrupts the signaling between your brain and your adrenals. The adrenal glands are still fully capable; they're just getting confused instructions. That distinction matters, because it changes where treatment should be focused: not on "supporting the adrenals" with a random supplement stack, but on restoring the rhythm and signal.

## What I Actually Do About It

Once I can see the actual shape of someone's curve, the plan gets much more specific than "manage your stress" (which, respectfully, is advice almost everyone has already tried and failed to act on).

- If the pattern is a flipped or flattened curve , the first priority is almost always circadian anchoring: consistent wake time, morning light exposure within the first 30-60 minutes of waking, and protein at breakfast instead of caffeine alone. This sounds almost too simple, but it is often the single highest-leverage change, because it's directly retraining the signal that sets the whole day's rhythm.
- If there's a midafternoon crash , I look closely at blood sugar stability,  what's happening at breakfast and lunch, and whether there's a pattern of skipping meals or relying on carbohydrate-heavy snacks that spike and then drop blood sugar, which the body reads as a stress signal in its own right.
- If DHEA is notably low relative to cortisol , we talk about training load, sleep debt, and sometimes targeted nutrient support; but only after the rhythm itself is being addressed, not as a substitute for it.
- In every case , I'm asking what's actually driving the demand on this system in the first place. Cortisol patterns are downstream of something , often a combination of sleep quality, blood sugar swings, unresolved gut inflammation, or a nervous system that's rarely getting a genuine "off" signal during the day.

## The Bottom Line

A normal cortisol result doesn't mean your stress response is working well, it often just means the test wasn't built to catch the kind of dysfunction you're actually dealing with. If you feel tired in the morning, wired at night, or like you're constantly running on a second wind you didn't ask for, that's real information, even if a single lab draw doesn't reflect it back to you.

The goal isn't to chase a "perfect" cortisol number. It's to understand your actual daily pattern and address what's driving it - so mornings feel less like a fight and nights feel like they actually let you rest.
