So Your Doctor Wants to Put You on a Statin? Here's What Your Cholesterol Panel Isn't Telling You

So your doctor just told you that you need to go on a statin.

Maybe your total cholesterol or LDL came back a little high. Maybe it's a routine screening and this is just "what we do" at a certain age. Either way, you're now holding a prescription and a lot of questions: Do I actually need this? What's really driving my risk? Is there something more specific going on that a standard panel just isn't picking up?

Here's the truth: a standard lipid panel is a good start, but it doesn't tell the whole story. Two people can have the exact same LDL number and very different actual cardiovascular risk, because LDL cholesterol measures the amount of cholesterol in your blood, not the number, size, and type of particles carrying it around, and it says nothing about inflammation or a separate, largely genetic risk factor most people have never been tested for.

That's where ApoB, Lp(a), particle size, and inflammation markers come in.

What is ApoB?

Every LDL particle, along with VLDL and other "bad" lipoproteins, carries exactly one ApoB (apolipoprotein B) molecule on its surface. That makes ApoB a direct count of the total number of artery-damaging particles in your blood, rather than just an estimate of how much cholesterol those particles are carrying. It's the particle itself that lodges in the artery wall and starts the plaque-building process, not the cholesterol content inside it.

What is Lp(a)?

Lipoprotein(a), or Lp(a), is a distinct particle structurally similar to LDL, but with an extra protein attached that makes it more prone to promoting both plaque buildup and clotting. It's almost entirely determined by genetics, doesn't move much with diet or lifestyle, and isn't part of a standard lipid panel. If it's elevated, it raises your cardiovascular risk on its own, independent of your LDL or ApoB numbers.

Why particle size matters

Not all LDL particles behave the same way. LDL comes in different sizes, commonly categorized as Pattern A (larger, more buoyant particles) or Pattern B (smaller, denser particles). Small, dense LDL particles are more easily able to squeeze into and damage artery walls, and they're more prone to oxidation, one of the key steps in plaque formation. So two people with an identical LDL cholesterol number can have very different risk profiles depending on whether their particles run large and buoyant or small and dense. An advanced panel measures LDL Particle Number, LDL Peak Size, LDL Pattern, and the breakdown of LDL Small, LDL Medium, and HDL Large, giving you a picture standard testing simply doesn't capture.

What about inflammation?

Cardiovascular risk isn't only about particles, it's also about what's happening in the artery wall itself. hsCRP (high-sensitivity C-reactive protein) is a marker of low-grade inflammation throughout the body, and chronic inflammation is a key driver of plaque instability, the process by which plaque ruptures and causes a cardiac event. Lp-PLA2 is a more vascular-specific inflammation marker, one that's been studied specifically for its association with plaque that's prone to rupture. Together, these two markers help show whether inflammation is actively contributing to your risk, something a standard lipid panel never measures at all.

Why this matters before you decide about a statin

A statin lowers LDL cholesterol. It can be a genuinely useful tool for some people. But if your real driver is a high ApoB particle count, small dense LDL particles, elevated Lp(a), or active inflammation, that decision deserves to be made with your actual numbers in hand, not just a total cholesterol or LDL result.
